Mandatory conditions where licence authorises the supply of alcohol
(1) No supply of alcohol may be made under the premises licence -
(a) at a time when there is no designated premises supervisor in respect of the premises licence, or
(b) at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
(2) The supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
1) "Over 21" policy in force at premises.
2) Visually inspect ID documents which would be a UK driving licence(photo part), Passport or Proof of Age Scheme Card.
3) Members of staff would be required to sign a sale of alcohol authorisation form when requests are made for proof of age.
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
1) CCTV system to be installed to address the prevention of crime objective. To be maintained and videos stored for 28 days.
2) External lighting to promote public safety objective.
3) No music be played at the premises.
4) All staff in the mini - market be under instruction from supervisor/personal licence holder to require photographic identity of anyone who looks underage and who tries to purchase alcohol.
