ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
1. CCTV maintained and operated at the premises both inside and outside the premises.
2. CCTV images retained for 31 days.
3. Fire safety procedure in place, including fire extinguisher, fire blanket and smoke alarm.
4. Illuminated fire exit signs maintained and all fire appliances and equipment maintained, tested and logged.
5. Clear signage displayed asking customers to leave quietly.
6. Challenge 25 posters to be clearly and visibly dispalyed at premises.
7. Staff to be trained to ask for ID where a customer appears to be under 25, and the only ID to be accepted are passports, driving licences or any other photographic document wth the PASS logo.
8. An ID log is to be kept to record dates and times where ID has been requested and to record the type of ID shown.
9. A copy of this ID log will be made available immediatley upon request to authorised officers of Rochdale Borough Council.
10. A refusals log is to be kept to record dates and times of all refusals to retail alcohol to underage persons.
11. A copy of the Refusals log will be made available immediatley upon request of authorised officers of Rochdale Borough Council.
12. To inform all staff of changes in the law relating to age-restricted products as and when required.
13. To notify staff of any reports of alleged underage sales incidents that has been brought to your attendtion by Trading Standards.
14. To keep a log of such notifications on file, ready for inspection by authorised officers of Rochdale Borough Council if required.
15. To record any remedial action following a report from Trading Standards of an alleged underage sale from the premises.
16. To produce a copy of the 'Record of Remedial Action' if requested by an authorised officer of Rochdale Borough Council.
17. Written records will be kept of all age verification training, with staff signing and dating records to confirm that the training has been recieved and understood.
18. No person under the age of 18 shall be permitted to enter or remain on the premises beyond 19:00hrs.
19.The premises licence holder is will adopt any additional, appropriate, and proportionate, operating conditions if requested by ANY Responsible Authority.
20. The Premises has a 24/7 mobile contact number working at all times to enable any Responsible Authority to make immediate contact with the PLH if required.
21. Digital hard drive CCTV system installed.
22. CCTV cameras cover internal and external areas of the premises and all areas where customers have legitimate access.
23. One CCTV camera is specifically positioned to capture facial images of any persons entering the premises.
24. All areas are sufficiently illuminated for the purposes of the recording of CCTV images.
25. All CCTV recorded images have sufficient clarity/quality/definition to enable facial recognition.
26. CCTV images are retained in an unedited format for a period of 31 consecutive days.
27. Any person left in charge of the premises is trained in the use of any such CCTV equipment and is able to produce/download/burn CCTV images.
28. CCTV is maintained on a regular basis and always kept in good working order and images shall be produced to the Police or Authorised Licensing Officers immediately upon request.
29. CCTV checked daily and if found not to be in good working order, alcohol will not be sold until such time as it is.
30. The CCTV system records images at all times when members of the public are at or on the premises.
31. Posters, no smaller than A4 size, are clearly and prominently displayed at the entrance and exit of the premises stating that CCTV is in operation and that images are being captured for the purposes of crime prevention and public safety.
32. A list of all employees is written and kept up to date showing names and dates of birth for each employee.
33. That list is provided to GMP / Licensing Authority immediately upon request.
34. The PLH will check and retain a copy of each employee UK Passport / and ensure their entitlement to work in the UK, prior to employment.
