ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
1. All staff employed have authority to reside and work in the UK.
2. Staff details will be maintained and made available to the Police and any other relevant authority on request.
3. Training provided to all staff upon induction on the sale of age restricted products before they serve behind any bar area. Records kept of training and made available to an authorised officer on request.
4. All staff notified of any reports of alleged underage sales incidents that have been brought to the attention of management by Trading Standards Service.
5. A log of such notifications kept on file and available on request of an authorised officer.
6. Remedial action recorded following a report of an alleged underage sale from the premises, a copy of record made available to an authorised officer on request.
7. All staff updated and informed of the changed in the law relating to the sake of age restricted products as and when required.
8. Challenge 25 Policy in place.
9. Posters, minimum size A4, displaying Challenge 25 Policy clearly and prominently displayed at relevant alcohol sale points throughout the premises.
10. Staff trained in regard to Licensing legislation and responsibilities under Challenge 25 Policies commensurate with their duties and responsibilities in the premises.
11. Training takes place at commencement of employment and every 12 months thereafter,
12. A record of training maintained and available to view by authorised authorities.
13. Refusals log maintained.
14. Refusal log produced on request to Police / Licensing Authority upon request.
15. Only forms of acceptable identification are: Photo driving license, Passport, Proof of Age Scheme Card which carries the PASS hologram and M.O.D identity cards.
16. No glass vessels will be used for the serving or consumption of alcohol on the premises.
17. Subscription to Town Centre Link Radio scheme.
18. Where SIA door securities are engaged at the premises, SIA staff will be supplied by a registered SIA company.
19. SIA registered door supervisors to work for a period of 6 months from opening of premises from 22:00 hours, until close of premises. Whilst alcohol is being served. (This condition can be reviewed following 6 month period and a variation to license submitted to remove SIA staff dependent upon any Crime and Disorder related incidents during that period).
20. On employment of SIA Staff, a register maintained recording the following details: SIA license number, license expiry date, name, duty commencement date/time, duty cessation time and the individual’s signature. This register made available to authorised officers on request.
21. Digital CCTV system installed and in operation.
22. CCTV retained in a secure environment.
23. CCTV system installed so as to ensure no ‘Black Spots’.
24. CCTV system shows correct time(s) and date(s)
25. CCTV cameras cover internal and external areas of the premises and all areas where customers have legitimate access (excluding toilets).
26. At least one CCTV camera specifically positioned to capture facial images of any person entering the premises at every entrance.
27. All areas sufficiently illuminated for the purpose of CCTV images.
28. CCTV images to be of evidential quality and capable of facial recognition
29. CCTV images retained for a minimum period of 31 consecutive days.
30. A person capable of operating the CCTV always available and able to produce, download or burn CCTV images if requested by Police or other relevant Licensing Authority.
31. CCTV maintenance carried out regularly and kept in good working order, records of these checks made and available for inspection by authorised officer on request.
32. CCTV checked weekly and if found not to be working, no alcohol will be sold until such time as it is.
33. Records of weekly checks to be made and available for inspection by authorised officer on request.
34. CCTV records at all times members of the public are on the premises.
35. Posters displayed in a prominent position at the entrances and exits of the premises notifying members of the public that CCTV is in operation and images are being captured for the purpose of crime prevention and public safety.
36. Displayed posters to be no smaller than A4 size.
37. Children under the age of 16 not permitted entry into the premises after 22:00 hours unless accompanied by someone over the age of 21.
38. Poster clearly and prominently displayed at point of entry detailing entry policy as regards age restrictions.
