ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
1. CCTV (positioned both inside and outside the premises) is to be recording and fully operational at all times the premises (off-licence and takeaway) are open to the public. Images recorded by the CCTV system shall be retained in unedited form for a period of not less than 28 days
2. CCTV footage to be given to officers of GMP and authorised officers of Rochdale Borough Council immediately upon their request
3. A refusals log detailing dates and times of refusals will be kept and maintained at the premises
4. A copy of the refusals log will be made available immediately upon request of authorised officers of Rochdale Borough Council
5. Any person who looks or appears to be under the age of 21 shall be asked to provide ID to prove that they are over the age of 18
6. Challenge 21 scheme to be operated at the premises
7. Challenge 21 posters to be clearly and prominently displayed within the shop
8. Only the UK photo driving licence, passport or government approved PASS cards will be accepted as proof of age
9. Training to be provided to staff, at least once a year to new and existing staff, on the law relating to underage sales of alcohol.
10.To keep written records of all training provided in respect of the law relating to underage sales of alcohol. Staff to sign and date records to confirm that the training has been received and understood
11. No alcohol delivery service to be provided
12. Any alcohol sold at the premises to be consumed off the premises only
13. Whenever the DPS is not at the premises, another person shall be nominated by them to be the responsible person to manage the premises
14. Litter surrounding the premises to be clearled on a daily basis
