ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
1. Tamper proof CCTV system to be installed, both inside and outside the premises and functioning fully during all hours that a licensable activity takes place on the premises
2. Recorded images show customers entering the store, show customers at the alcohol displays and provide clear uninterrupted facial images of customers at the point(s) of sale
3. Signage on entry to the store clearly stating CCTV in operation
4. CCTV equipment to be maintained in good working order in accordance with manufacturers instructions, checked each day, and if found not to be working no alcohol is to be sold until such time that it is
5. Images recorded by CCTV system shall be retained in unedited form for a period of not less than 31 days
6. CCTV footage to be given to officers of GMP and authorised officers of Rochdale Borough Council immediately upon their request
7. A member of staff capable of downloading the images is present at all times
8. Challenge 25 scheme to be operated at the premises
9. Challenge 25 posters to be clearly and prominently displayed within the shop
10. A refusals log detailing dates and times of refusals to be kept and maintained at the premises
11. A copy of the refusals log to be made available immediately upon request of authorised officers of Rochdale Borough Council
12. Any person who looks or appears to be under the age of 25 shall be asked to provide ID to prove they are over the age of 18
13. Only the UK photo driving licence, passport or government approved PASS cards will be accepted as proof of age
14. Training to be provided to staff, at least once a year to new and existing staff, on the law relating to underage sales of alcohol
15. Staff to be informed of changes in the law relating to age-restricted products
16. To keep written records of all training provided in respect of the law relating to underage sales of alcohol. Staff to sign and date records to confirm that the training has been received and understood
17. Staff to be notified of any reports of alleged sale incidents that have been brought to their attention by Trading Standards
18. Log to be kept of such notifications on file, ready for inspection by authorised officers of RMBC
19. To record any remedial action following a report from Trading Standards of an alleged underage sale from the premises
20. To produce a copy of the "Record of Remedial Action" if requested by an authorised officer of RMBC
21. Any alcohol sold at the premises to be consumed off the premises only
22. Wherever the DPS is not at the premises, another person shall be nominated by them to be the responsible person to manage the premises
23. Litter surrounding the premises to be cleared on a daily basis
