ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
1. All orders of alcohol for delivery will be telephone / internet booking or by prior arrangement.
2. All payments to be taken over the telephone via credit or debit card.
3. No cash to be exchanged by customer to driver or visa versa.
4. Alcohol will only be delivered to persons over 18 years of age and proof of age will be requested where persons appear to be under 21 years of age.
5. A log is to be kept of all deliveries and this is to be available to the Police and Rochdale MBC Authorised Officers upon request.
6. All vehicles and VRM's used for deliveries are to be communicated GMP.
7. No other vehicle is to be used until GMP is made aware.
8. DPS to inform GMP of all employees and delivery drivers - all changes to be communicated within 24 hours.
9. All delivery staff to be trained by a Personal Licence Holder:
a) When leaving car at customers address, always leave car full locked with order inside.
b) If customer appears under 21; always demand identification. Valid passport or UK driving licence (photo card), a recognised Proof of Age card.
c) Do not ask help of customer to carry order in.
d) Unlock car, only bring out the specific order for that customer.
e) Always deliver to door.
f) At any point if you are uncomfortable; drive off or ring for assistance.
10. Health and Safety legislation complied with.
11. All drivers will be instructed to keep any noise to a minimum between 23:00 and 06:00 hrs.
12. CCTV system must be maintained and fully operational. CCTV to be installed internally and externally.
13. Images are to be stored for a period of not less than 31 days.
14. CCTV to record at all times the premises are open.
15. Images are to be made available to any Police Officer or Authorised Officers of Rochdale MBC Licensing upon their request.
16. A policy detailing training and rules provided to delivery staff is to be devised by the Premises Licence Holder. The policy is to be agreed with the Police.
17. To ask for appropriate identification of proof of age with a photograph of the person displayed in the ID.
18. Record this request on an ID Request Log.
19. To provide training, at least once a year, to all existing staff on age restricted products.
20. To keep a written record of all training provided and get staff to sign and date records to confirm that the training has been received and understood.
21. To notify staff of any reports of alleged underage sale incidents that has been brought to your attention by Trading Standards.
22. To keep a log of such notifications on file, ready for inspection by authorised officers of RMBC if required.
23. To record any remedial action following a report from Trading Standards of an alleged underage sale from the premises.
24. To produce a copy of the ' Record of Remedial Action' if requested by an authorised officer of RMBC.
25. Children will not be allowed to enter the premises at anytime.
26. Members of the public will not have access to the premises.
27. Out door lighting will be installed.
28. A fully trained member of staff will be at the entrance door to the premises at all times during the hours of operation.
29. All bottles containing alcohol will possess a unique identifiable serial number on them.
